Deed of Revocation for a Living Will

A Living Will is a Will designed to provide only for specific circumstances that may occur during your lifetime. Its purpose is to set out  information regarding your health care and medical treatment choices such as life support if you become unable to communicate as a result of a serious injury or illness. For example, as part of your Living Will you may state that you do not want to be resuscitated.

The idea of a Living Will is to help your family make medical decisions based on your wishes. If you are no longer able to speak  or make decisions for yourself you will need a document to do this for you.

A Revocation of a Living Will is for use if you change your mind about all or part of the information in the Living Will. For example ,you may have put in your Living Will that you wish your organs to be donated upon your death, but as you age you may decide you do not want this. The Revocation document will void the Living Will you created so that it no longer is enforceable.
